Short answer: a marketplace plan comes from your home state, and on most of them a clinic three states away is covered only if it is an emergency. Owner-operators over the tax-credit line pay the full price for that. A private plan on the UnitedHealthcare or PHCS PPO network has providers along every route, and the main one has no deductible.
A sinus infection in Ohio, a sprained wrist at a dock in Georgia, a refill in Arizona: none of those is an emergency, so an HMO or EPO from your home state will usually not pay for them. Tennessee’s marketplace was all EPOs in 2026; Texas, Ohio, Indiana, Kansas and Utah had no PPO either.
Age 45 in Tennessee, before health questions. The private plan covers less than an ACA plan and asks about your health; the marketplace price is before any tax credit.
More for drivers: health insurance for truck drivers, by county.

The insurance company pays the agent, never you, and the premium is the same either way.

A plan with a national network: a Blue Cross PPO with BlueCard where one is sold, or a private plan on the UnitedHealthcare or PHCS PPO network. A home-state marketplace HMO or EPO covers other states only in emergencies.
Yes. Every ACA plan must cover emergency care anywhere, without prior approval and at in-network cost-sharing. Routine and follow-up care is where HMOs and EPOs stop.
Self-employed health insurance premiums are generally deductible on IRS Form 7206. Ask your tax advisor how it applies to your plan.
General information, not a quote or an offer of coverage. Private plans are medically underwritten: they ask health questions, can decline or exclude a condition, are not ACA coverage and get no tax credit. “I run my own business and my time is valuable. Daniel handled everything — compared the private options, explained what I was actually getting, and had me covered fast. Way better experience than trying to navigate it myself.”
